The Sugar Connection: How Diabetes and Insulin Resistance Affect the Brain

In my practice, I often see patients who are concerned about memory loss but have never been told that their blood sugar levels could be affecting their brain. The connection between diabetes, insulin resistance, and Alzheimer's disease is so strong that some researchers have referred to Alzheimer's as "Type 3 diabetes." While the term is not entirely precise, it captures a profound truth: the brain is not separate from the body's metabolic health.

The Science: Insulin and the Brain

Most people think of insulin as the hormone that regulates blood sugar. But insulin does much more than that. In the brain, insulin plays a crucial role in memory, learning, and synaptic plasticity — the brain's ability to form and strengthen connections. Brain cells depend on insulin to absorb glucose, their primary fuel source.

When insulin resistance develops — meaning that cells no longer respond efficiently to insulin — the brain is affected in two ways. First, brain cells receive less glucose, which impairs their ability to function. Second, insulin resistance triggers inflammation and oxidative stress, both of which damage neurons and contribute to the accumulation of amyloid and tau proteins, the hallmarks of Alzheimer's disease.

What the Data Tells Us

  • A meta-analysis published in Diabetes Care found that people with type 2 diabetes had a 60% higher risk of developing Alzheimer's disease compared to those without diabetes.
  • A 2023 study in Neurology followed over 10,000 participants and found that elevated blood sugar levels — even in the "prediabetic" range — were associated with faster cognitive decline over time.
  • Research published in The Lancet Diabetes & Endocrinology found that optimal blood sugar control in midlife was associated with significantly better cognitive function decades later.
  • A 2024 study in JAMA Network Open found that treating insulin resistance with medication was associated with a slower rate of cognitive decline in individuals with mild cognitive impairment.

These studies point to a clear and actionable conclusion: managing blood sugar is brain protection.

 

What I Recommend to My Patients

1. Know Your HbA1c

HbA1c is a blood test that measures your average blood sugar over the past two to three months. It is a far better indicator of metabolic health than a single fasting glucose reading. Know your number. If it is elevated, take it seriously — not just for your heart, but for your brain.

2. Move After Meals

A short walk — even just 10-15 minutes — after eating significantly blunts the post-meal blood sugar spike. This is one of the simplest, most effective interventions available. It requires no equipment, no prescription, and no special skills. Just movement, timed strategically.

3. Reduce Refined Carbohydrates and Sugary Beverages

White bread, white rice, sugary drinks, and processed snacks cause rapid blood sugar spikes. Over time, these spikes contribute to insulin resistance. Replacing refined carbohydrates with whole grains, legumes, and vegetables helps stabilize blood sugar and support brain health. You do not need to eliminate carbohydrates entirely; you need to choose them wisely.

4. Prioritize Protein and Fiber at Every Meal

Eating protein and fiber alongside carbohydrates slows glucose absorption and reduces blood sugar spikes. Even a small adjustment — adding an egg to your toast, or a handful of nuts to your fruit — makes a measurable difference.

5. Treat Sleep Apnea

Sleep apnea, which is common in people with diabetes, fragments sleep and worsens insulin resistance. Treating it with CPAP or other interventions can improve both metabolic and cognitive health. If you snore heavily, feel unrefreshed after sleep, or have been told you stop breathing at night, discuss a sleep study with your physician.

 

A Word to Caregivers

Caregiving is stressful, and stress raises blood sugar. You may find yourself grabbing whatever food is quickest, skipping meals, or relying on sugar and caffeine to keep going. This is understandable — but over time, it takes a toll.

Please hear this: protecting your metabolic health is not a luxury. It is a strategy for sustaining the energy, clarity, and resilience you need to keep caring for the person you love. One small choice at a time — a walk after a meal, a glass of water instead of soda, a piece of fruit instead of a candy bar — adds up.

Your brain needs you to take care of your body. And your body is the instrument through which you give care. Protect it.
