As a neurologist, one of the most common complaints I hear from my patients is not about memory loss. It is about sleep.

"I can't fall asleep." "I wake up at 3 a.m. and can't go back to sleep." "I sleep eight hours, but I still feel exhausted."

If you are a caregiver, this probably sounds familiar. You lie in bed, exhausted from the day, but your mind refuses to shut down. You replay conversations. You worry about tomorrow. You stare at the ceiling, waiting for the alarm to go off.

But here is what I want every caregiver to understand: poor sleep is not just a symptom of your stress. It is a direct threat to your brain health — and to the brain of the person you are caring for.

 

The Science: What Happens in Your Brain While You Sleep

Sleep is not a period of brain inactivity. It is a period of intense, critical brain maintenance.

During deep sleep, your brain activates a remarkable self-cleaning system called the glymphatic system. Think of it as a janitorial crew that works the night shift. This system flushes out metabolic waste products that accumulate in the brain during waking hours — including beta-amyloid, the sticky protein that forms the plaques found in Alzheimer's disease.

This discovery earned significant scientific attention and has reshaped how we understand sleep and neurodegeneration. A landmark study published in Science demonstrated that during sleep, the space between brain cells increases by up to 60%, allowing cerebrospinal fluid to flow through and clear away these toxic byproducts far more efficiently than during wakefulness.

In short: every night of good sleep is a night your brain spends cleaning itself. Every night of poor sleep is a missed cleaning cycle.

 

What the Data Tells Us

The epidemiological evidence is equally compelling. Multiple large-scale studies have found that:

  • People who consistently sleep less than 6 hours per night in midlife have a 30% higher risk of developing dementia later in life, compared to those who sleep 7-8 hours.

  • Chronic sleep fragmentation — waking up repeatedly throughout the night — is associated with faster cognitive decline, even in people who spend enough total time in bed.

  • A study published in Nature Communications found that people in their 50s and 60s who slept six hours or less per night had a significantly higher burden of beta-amyloid in their brains decades later.

Sleep is not a luxury. It is a biological necessity for brain preservation.

 

Why Caregivers Are Particularly at Risk

I want to speak directly to the caregivers reading this.

You are at the intersection of multiple sleep-disrupting forces. Stress. Hypervigilance. Irregular schedules. Emotional exhaustion. You may feel that sacrificing your sleep is part of the job — a necessary cost of loving someone through this disease.

Please hear me: sacrificing your sleep is sacrificing your brain health. And if your brain health declines, your ability to care for your loved one declines with it.

This is not selfishness. It is strategy. Protecting your sleep is one of the most powerful preventive actions you can take — for yourself, and for the person who depends on you.

 

What I Recommend to My Patients

Improving sleep does not require a complete life overhaul. Start with one or two of these evidence-based strategies:

1. Set a Non-Negotiable Wind-Down Window

Your brain needs a transition period between the stress of caregiving and the state of rest. For at least 30-60 minutes before bed, step away from screens, dim the lights, and engage in something calming — a warm bath, soft music, quiet reading. This is not wasted time. It is essential brain preparation.

2. Anchor Your Wake-Up Time

Going to bed at the same time every night is hard for caregivers. But you can control your wake-up time. Waking up at the same time every day — even on weekends — strengthens your circadian rhythm. Over time, this makes it easier to fall asleep at a consistent hour.

3. Treat Sleep Apnea — Yours and Theirs

Sleep apnea, a condition where breathing repeatedly stops and starts during sleep, is a major risk factor for cognitive decline. It is also treatable. If you or your loved one snores loudly, gasps during sleep, or wakes up feeling unrefreshed despite enough hours in bed, discuss a sleep study with your physician.

4. Limit Alcohol Before Bed

Many caregivers use a glass of wine to unwind. I understand why. But alcohol is a sleep disruptor. It may help you fall asleep faster, but it fragments your sleep in the second half of the night — precisely when your brain needs to do its deepest cleaning. If you drink, try to finish at least three hours before bedtime.

 

A Final Word

I know that asking a caregiver to "sleep better" can feel almost insulting. You have so little control over your nights. A loved one may wander. They may call out. They may need you at unpredictable hours.

But I am not asking you to be perfect. I am asking you to treat sleep as a priority — as important as the medications you manage and the appointments you keep. Because it is.

Every hour of restful sleep is an investment in your cognitive resilience. Every night of good sleep is a small victory in the fight to protect your brain.
