Move to Remember: How Physical Activity Protects the Aging Brain

If there were a prescription that could cut your risk of Alzheimer's by nearly half, would you take it?
What if that prescription didn't come in a bottle? What if it was free, available to almost everyone, and had no dangerous side effects?
That prescription exists. It is physical activity. And the evidence behind it is now so overwhelming that I consider exercise to be one of the most powerful preventive tools we have against cognitive decline.
The Science: What Exercise Does to Your Brain
When you exercise, you are not just training your muscles. You are training your brain.
Physical activity increases blood flow to the brain, delivering oxygen and nutrients essential for neuronal health. It stimulates the release of brain-derived neurotrophic factor (BDNF) — a protein often described as "Miracle-Gro for the brain." BDNF promotes the survival of existing neurons, encourages the growth of new ones, and strengthens the connections between them, particularly in the hippocampus, the brain's memory center.
The hippocampus is one of the first regions to shrink in Alzheimer's disease. A landmark study published in the Proceedings of the National Academy of Sciences found that older adults who walked briskly for 40 minutes, three times a week, for one year showed a 2% increase in hippocampal volume. The control group, who only did stretching and toning exercises, showed the expected age-related decline.
Let that sink in. Exercise did not just slow brain shrinkage — it reversed it.
What the Data Tells Us
The epidemiological evidence is equally compelling:

A meta-analysis published in Neurology pooled data from over 160,000 participants and found that regular physical activity was associated with a 45% reduction in the risk of developing Alzheimer's disease.

A study published in The Lancet followed nearly 500,000 participants and found that those who met the World Health Organization's minimum physical activity guidelines — 150 minutes of moderate activity per week — had a 20% lower risk of dementia compared to those who did not.
These are not small, uncertain effects. These are consistent, reproducible findings across diverse populations and study designs. The message is clear: the physically active brain ages slower.
What I Recommend to My Patients
You do not need a gym membership. You do not need expensive equipment. You need consistency, not intensity.
1. Start with 30 Minutes of Brisk Walking, Five Days a Week
This is the most accessible, sustainable, and evidence-backed form of exercise for brain health. A brisk walk — where you can still talk but cannot sing — is enough. If 30 minutes feels like too much, start with 10. The goal is to build a habit, not to exhaust yourself.
2. Add Resistance Training Twice a Week
Muscle-strengthening activities are not just for your body. Emerging research suggests that resistance training also improves executive function and memory. Bodyweight exercises — squats, push-ups, lunges — count. Resistance bands count. Lifting grandchildren counts.
3. Find Something You Enjoy
The best exercise for your brain is the one you will keep doing. Dancing, swimming, cycling, tai chi, yoga, hiking — these all provide the cardiovascular stimulus your brain needs. Choose something that brings you joy, and it will not feel like medicine.
4. Move Throughout the Day, Not Just in One Block
Prolonged sitting is an independent risk factor for poor health outcomes, including cognitive decline. If you spend long hours at a desk or beside a loved one's bed, set a timer to stand up and walk for two minutes every hour. These small movements accumulate and matter.
A Word to Caregivers
I know you are exhausted. I know the idea of "exercising" can feel like yet another demand on a body and mind that are already giving everything they have.
But hear me: this is not another thing you have to do for someone else. This is something you do for you. A ten-minute walk around the block is not selfish — it is a strategic investment in the brain you need to keep showing up for the person who depends on you.
Start small. Start slow. But start. Your brain will thank you for decades to come.
