In my clinic, I often ask patients about their exercise habits. The responses vary: "I walk when I can." "I'm too tired." "I used to be active, but life got in the way."
For caregivers, finding time to exercise can feel impossible. Your day is not your own. Your energy is spent on someone else. The idea of adding "workout" to your already overwhelming schedule feels almost disrespectful.
But here is what I want you to know: physical activity is not a luxury you can't afford. It is a neuroprotective necessity you cannot afford to skip.
The Science: Exercise as Brain Medicine
Over the past two decades, a wealth of research has established physical activity as one of the most powerful interventions for brain health. The mechanisms are multi-layered and deeply compelling:

Increased Blood Flow: Exercise boosts cerebral blood flow, delivering oxygen and nutrients to brain cells while removing waste products. This is especially important for the hippocampus — the brain's memory center, and one of the first regions affected by Alzheimer's.

Brain-Derived Neurotrophic Factor (BDNF) : Often called "Miracle-Gro for the brain," BDNF is a protein that supports the survival of existing neurons and encourages the growth of new ones. Aerobic exercise is the single most effective way to raise BDNF levels naturally.

Reduced Inflammation: Chronic inflammation is a known contributor to cognitive decline. Physical activity reduces systemic inflammation, including in the brain, creating a less hostile environment for neural cells.

What the Research Shows
The evidence is not just theoretical. Large-scale studies have consistently demonstrated the protective effects of exercise on the aging brain:

What Kind of Exercise? How Much?
You don't need to become a marathon runner to benefit. The most effective exercise regimen for brain health has three components:
1. Aerobic Exercise — 150 Minutes Per Week
Brisk walking, swimming, cycling, dancing — anything that gets your heart rate up. Aim for 150 minutes per week, which breaks down to just 30 minutes, five days a week. If 30 minutes feels impossible, start with 10. Even short bursts of activity count.
2. Strength Training — Twice Per Week
Muscle-strengthening activities are not just for bodybuilders. Resistance exercises — using weights, resistance bands, or your own body weight — improve insulin sensitivity and stimulate BDNF production. Two sessions per week, focusing on major muscle groups, is sufficient.
3. Balance and Coordination Exercises
Tai chi, yoga, and simple balance drills reduce fall risk and engage the brain's motor control centers. These activities also provide a mindful, meditative quality that reduces stress — an added benefit for brain health.

I want to speak directly to you: the person reading this while sitting beside a loved one, or in the few quiet minutes before the next task calls.
Exercise can feel like one more thing on a list that is already too long. But you are the most important tool in your caregiving toolbox. Your brain is your greatest asset. Protecting it is not selfish — it is the single most strategic thing you can do for the person who depends on you.
Find small moments. Walk up and down the driveway while your loved one naps. Do chair exercises in the room beside them. Put on music and dance together — movement is medicine for both of you.
You are not asking for permission to take care of yourself. You are giving yourself permission. There is a difference.
How to Start — Today

Go for a 10-minute walk. Right now, if you can. Around the block. Down the hallway. Just move.

Find something you enjoy. The best exercise is the one you'll actually do. Gardening, dancing, hiking, swimming — they all count.

Buddy up. Exercise with your loved one, a friend, or join a group. Social connection amplifies the brain benefits.

Track it. Use a simple calendar or a fitness app. Checking off each day builds momentum and reinforces the habit.
Your brain is counting on you to move. Start where you are. Do what you can. But start.
