When I see a patient concerned about their memory, I don't just examine their brain. I ask about their blood pressure. I review their cholesterol levels. I discuss their exercise habits, their diet, and whether they smoke.
This surprises some people. They came to talk about Alzheimer's disease, not heart disease. But the two are far more connected than most realize. In fact, what is good for the heart is almost always good for the brain. And understanding this connection can open up powerful avenues for prevention.
The Science: A Shared Circulatory System
The brain is a voracious consumer of oxygen and nutrients. Despite weighing only about 2% of your body mass, it receives approximately 15-20% of your cardiac output. Every heartbeat sends blood surging through an intricate network of arteries, capillaries, and veins that supply your neurons with everything they need to function.
When that vascular system is compromised, the brain suffers. High blood pressure damages the delicate walls of small blood vessels in the brain, leading to microbleeds, white matter lesions, and reduced cerebral perfusion. High cholesterol contributes to atherosclerosis, narrowing the arteries that feed the brain. Diabetes, through its effects on blood vessels and insulin signaling, increases the risk of both vascular dementia and Alzheimer's disease.

- A meta-analysis published in The Lancet Neurology reviewed data from over 1.3 million participants and identified hypertension, obesity, diabetes, and smoking as among the most consistent modifiable risk factors for dementia. Addressing these factors could theoretically prevent or delay up to 40% of dementia cases worldwide.
- A 2023 study in JAMA Network Open found that treating hypertension aggressively — bringing systolic blood pressure below 120 mmHg — resulted in significantly less white matter damage on brain scans compared to standard treatment targets.
- Research on statins, the cholesterol-lowering medications, has shown that they may reduce the risk of developing Alzheimer's disease by as much as 28% in some populations, likely through their anti-inflammatory and vascular protective effects.
What I Recommend to My Patients
1. Know Your Numbers
You cannot manage what you do not measure. Know your blood pressure. Know your cholesterol levels, including LDL, HDL, and triglycerides. Know your fasting blood sugar and HbA1c. These numbers are not just abstract lab values; they are powerful predictors of your brain's future health. Write them down, track them over time, and discuss them with your physician.
2. Treat Hypertension Aggressively
If you have high blood pressure, take it seriously. Lifestyle changes — reducing sodium, increasing potassium-rich foods, maintaining a healthy weight — are foundational. But do not hesitate to use medications when they are needed. The brain benefits of blood pressure control appear to be substantial and long-lasting.
3. Move Your Body — and Your Blood
Aerobic exercise is one of the most effective ways to lower blood pressure, improve cholesterol profiles, and enhance cerebral blood flow. Brisk walking, swimming, cycling, or dancing for 30 minutes most days of the week is a prescription that benefits both heart and brain simultaneously. It also happens to be one of the few interventions shown to increase hippocampal volume.
4. Quit Smoking and Limit Alcohol
Smoking damages blood vessels throughout the body, including in the brain. The evidence is unequivocal: quitting smoking at any age reduces your risk of stroke and dementia. Alcohol, in excess, raises blood pressure and contributes to brain atrophy. If you drink, do so moderately — or consider stopping altogether.
5. Treat Atrial Fibrillation and Other Arrhythmias
Atrial fibrillation (AFib), an irregular heartbeat that becomes more common with age, significantly increases the risk of stroke and silent brain injuries. Many people with AFib are unaware they have it. If you experience palpitations, an irregular pulse, or unexplained episodes of dizziness, discuss evaluation with your physician. Properly managed AFib dramatically reduces stroke risk.
A Final Word
The boundary between "heart disease" and "brain disease" is artificial. They share the same vasculature, the same risk factors, and increasingly, the same preventive strategies. Protecting your heart is protecting your brain. And protecting your brain begins long before any symptom of cognitive decline appears.
The steps outlined in this article are not optional extras. They are among the most well-established, evidence-based actions you can take to reduce your risk of dementia. And they are largely within your control.
